Description

This training session focuses on the fundamental concepts underlying arterial blood gas (ABG) analysis. It covers acid-base physiology, pH measurement, blood gas analyzer technology, carbon dioxide measurement, regulation of hydrogen ion concentration, buffer systems, and the physiological responses to acid-base disturbances. The session further explores the effects of anaesthesia on acid-base balance and the systemic consequences of acid-base disorders.

Objectives

Define acids, bases, and conjugate acid-base pairs using the Brønsted-Lowry theory. Explain the concept of pH and its relationship to hydrogen ion concentration in blood. Describe the principles of pH measurement and the operation of the glass electrode used in blood gas analyzers. Explain the measurement of carbon dioxide using the Severinghaus electrode and its clinical relevance. Discuss the normal regulation of hydrogen ion concentration and the roles of buffers, ventilation, and renal mechanisms in acid-base homeostasis. Describe the bicarbonate buffering system and the interaction between the lungs and kidneys in maintaining acid-base balance. Assess how anaesthesia and ventilation changes affect acid-base status, including the development of respiratory acidosis and alkalosis. Identify the major physiological buffer systems, including bicarbonate, haemoglobin, protein, phosphate, and ammonia buffers. Explain renal compensation mechanisms in maintaining acid-base equilibrium. Describe respiratory compensation mechanisms in response to metabolic acid-base disturbance. Recognize the systemic effects of acid-base disorders on the cardiovascular, respiratory, neurological, electrolyte, and cellular metabolic systems.
